Home Care Coordination and Consulting is where a social worker and/or geriatric care manager collaborates with a person to help someone age in place. To do that, they familiarize the person with the care they need to do that. They can help the person develop a plan of care for their needs and situation. 

Based on the assessment, the social worker or geriatric care manager provides the person with care options. They assess the individual’s risk factors such as home safety check and fall prevention. They facilitate referrals to appropriate support services. These services can help ensure the person’s wellbeing and safety.

Once they’ve gone through this process, the person can determine if they want to bring the recommended services into their house. These services can help ensure the wellbeing and safety of an individual. Why is this important? The individual may want to live on their own. The individual’s children may think they should be in an assisted living facility. 

With this analysis, the individual’s children may feel reassured that, with certain providers in place, it is safe for the individual to remain in their home. That way, the individual is happy that they can remain in their home. The individual’s children may now be content that the individual is being cared for appropriately.
